Medical Records

Medical records cannot be released without proper written authorization from the patient as defined by the Health Insurance Portability and Accountability Act (HIPAA). Records are copied and released once per week.  A fee is assessed in accordance with the State of Alabama Reproduction Code Section 12-21-6.1.  The reasonable costs of reproducing copies of written or typed documents, or reports shall not be more than:

  • $1 for each page of the first 25 pages
  • Not more than $.50 for each page after the first 25 pages
  • A $5 search and handling fee
  • If the medical records are mailed to the person making the request, reasonable costs shall include the actual costs of mailing the medical records
  • A person may be charged, in addition to the above fees, the actual cost of reproducing X-rays and other special medical records

Other forms, such as disability, FMLA, or other related forms are also only completed once a week.  There is a $10.00 charge for the process and completion of such forms.

Release of Information

In accordance with the Health Insurance Portability and Accountability Act (HIPAA), patients must complete A General Authorization for Use and Disclosure of Protected Health Information in order for us to disclose personal information to any third party, including family members. It is the patient's responsibility to update this form if any of the information changes. All patients are required to complete this form. If you have not already completed it prior to your visit, you will be asked to complete the form.  You will also receive a copy of our PRIVACY NOTICE.
