We advise all patients to follow the steps below to help us meet their needs quickly and effectively:
- Please call us at (205) 787-8676 for appointments in advance.
- If you are unable to make it to your appointment for any reason, please notify us as early as possible.
- Please notify our Billing Department about any unusual financial situations that may create difficulties for you in paying your bill.
- For Worker’s Compensation, please confirm that each appointment and/or scheduled test has been approved by your case manager.
- Remember to notify us of any changes in your insurance coverage as they occur.
- Please inform us about changes in address, telephone number, name, etc.
- Bring your insurance cards and a valid driver's license or photo ID to all your appointments.
- Note that all co-payments are due at the time of service.
- If you do not have your referral at the time of your appointment, we will need to reschedule your appointment until the referral is made. Alternatively, you may pay for services rendered during the visit.
- When requesting medication, it is recommended that you have your pharmacy phone number ready. Please note that we cannot call in any pain medications to the office due to federal regulations.
